What Is Form N-648?

Form N-648 is used by permanent residents applying for U.S. naturalization who are unable to meet the English language and/or civics testing requirements due to a documented medical or psychiatric condition.

Call 913-787-5805 or email [email protected]. We will email you a Permission form for you to complete, scan, and email back. We will then call you to set up your telemedicine consultation with Dr. Gordon.

The Purpose of Your Telemedicine Consultation

To confirm your identity
To clarify the reason that you believe qualifies you for a medical exception to policy for the English language and/or civics test requirements for naturalization
To undergo a mental status and physical exam to the extent possible via telemedicine

Cost: $520 by credit card in advance

What to Have Available for Your Telemedicine Consultation

Your United States Permanent Resident card (green card).
A translator/personal advocate who is fluent in English (speaking, understanding, reading, and writing) and your native language must be present. This may be a professional, family member, or friend.

After Your Telemedicine Consultation

Our office will contact your personal physician to request a letter/medical record summary in support of your claim. Your N-648 cannot be accomplished without this.
If you do not have a personal physician in the United States, Dr. Gordon will let you know the specialist you would have to consult.

Expedited Service is a premium option where your case is given top priority, with all steps and communications handled as if you were our only client.

An expedite request is a special request to have your case processed faster than usual, using our Expedited Service option.

Expedited Service is typically used for tight deadlines or urgent situations. Availability is limited, so it’s best to inquire when scheduling your appointment.

On average, cases using Expedited Service are completed in about 3 to 5 calendar days - roughly half the usual processing time.

Expedited Service requires an additional $200 fee for priority handling.
